Adenomyosis: Painful and Disturbing

 

Before we understand what adenomyosis is, there is one part of the woman’s body one should be familiar with.  It is her uterus.

The uterus is a muscular organ which is hollow and is located between the urinary bladder and the rectum in the female pelvis.



 

It is pear-shaped and is divided into four regions i.e. fundus (which is the upper-most rounded portion of the uterus), corpus (body), cervix, and cervical canal.

 

 



 

There are three layers of the uterus, namely, endometrium, myometrium, and perimetrium. These three layers form the uterine wall in a normal uterus. The endometrium is the innermost layer and is made up of epithelial cells.  Myometrium is the middle layer and consists of smooth muscles. The perimetrium is the outer-most serous layer of visceral peritoneum.

 

 



 

 

This description of the normal uterus will help us understand the changes that occur in adenomyosis. In adenomyosis, the cells that make the endometrium lining of the uterus are atypically present inside the myometrium lining as the basal endometrium penetrates into myometrial fibers.

 



 

Causes:

The exact cause of adenomyosis is unknown.

Adenomyosis has been found to be initiated when a condition is able to break the junctional zone or the barrier between endometrium and myometrium.

The break in the barrier can be caused by any uterine trauma including cesarean section, surgical termination of pregnancy, or any kind of pregnancy.

 

The women are more likely to suffer from adenomyosis include the women of reproductive age. However, younger women can also suffer from adenomyosis. In many women, it gradually resolves after menopause.

 

Symptoms and Signs of Adenomyosis:

In some women, i.e. in nearly 33% of women suffering from adenomyosis, adenomyosis is completely asymptomatic. In some cases, adenomyosis is severely debilitating. The symptoms of adenomyosis are first reported between the ages of 40 to 50 years but younger women can also report symptoms.

The mostly presented symptoms and signs include-

·        Severe and chronic pelvic pain: The severity of pelvic pain is more in adenomyosis as compared to that in fibroids, generally.

·        Heavy periods: It is more common when adenomyosis is deeper and can lead to anemia, dizziness, fatigue, and moodiness. Bleeding is generally more in fibroids.

·        Abnormal uterine bleeding: Bleeding can occur in between periods.

·        Pain during periods: There can be severe pain before and during the periods.

·        The bladder may feel pressurized as the size of the uterus grows and it becomes bulky.

·        There may be ‘a bearing down’ feeling. There may also, be a dragging sensation that goes down the thighs and legs.

·        Enlargement of the uterus creates a pelvic fullness.

·    The uterus becomes tender.

·        Adenomyosis is associated with infertility, and sub-fertility in which there is 000an increased risk of pre-term labor and premature rupture of membranes.

 

Diagnosis:

Transvaginal ultrasonography (TVUS) and magnetic resource imaging (MRI) can be used to diagnose adenomyosis.

Management:

In conventional medicine, surgical removal of the uterus can be done. The patients are prescribed pain-killers and hormonal pills which have limited action.

In Homeopathy, gradual treatment can help in lowering pain, menstrual bleeding, and can help in decreasing the intensity of other associated symptoms such as painful sexual intercourse.

Homeopathy can limit the intake of conventional pain-killers. Each homeopathic remedy for patients with adenomyosis is selected on the basis of the personal and medical history of the patient, keeping in mind the medical history of the family as well. There are remedies like Sabina, Thlaspi which help in the management of adenomyosis but none shall be taken without the prescription of a homeopathic doctor.

The selection of remedy also depends on the type, color, and amount of bleeding, the intensity of pain, the factors which increase and decrease the pain and bleeding. Personal likings and disliking also define the selection of a homeopathic remedy.

It shall also be kept into consideration that the effect of homeopathic remedies may vary from person to person. While some females experience relief within the first two months of homeopathic treatment, others may take a while longer to experience relief.
